If you receive 1099 forms, due to the recent acquisition of Summa Health by General Catalyst’s Health Assurance Transformation Company (HATCo), you will be receiving two forms for 2025. One will indicate Summa Health and the other will indicate Akron Assurance Hospital Company. Per guidelines, 1099 forms will only be issued if payments in 2025 were greater than $600. These forms will be mailed to the address SummaCare has on file by January 31, 2026.
