Within the next week or so, our mutual Chamber Health Benefits Plan (MEWA) groups will
receive an email request from mewa.io, the MEWA management system used to collect data
for the Annual Rate and Census Reconciliation (ARC).

Consoliplex, the plan manager, will then use this data to file their IRS Form 5500 SF. The collection period will run from April 3 to May 12, 2023. It is very important that the groups provide the most accurate data for this IRS
reporting purpose. Steve Millard, president of Greater Akron Chamber, will be emailing all groups as well to assure them this is a legitimate email to alleviate any concerns over spam, phishing and other cyber data issues that are unfortunately very prevalent.

Many of you are already familiar with this process for other MEWA insurance carriers.
Brokers/agents with a mewa.io Broker Account can check on the completion status of their
groups as well as complete the form on behalf of their client groups.
