The Consolidated Appropriations Act (CAA), 2021, requires the annual reporting of certain plan
information such as plan 5500 number(s) and split of premium paid between the employer and the
employee.
Medical Mutual will reach out to Small (1-50) and Midsize and Large (51+) groups in early February 2024 to ask them to complete a short, online survey to collect 2023 plan year information; some Midsize and Large Group cases may be asked to supply 2024 plan year information (details below). All groups are asked to complete the online survey by Friday, March 1, 2024. If they do not complete the survey by March 1, Medical Mutual cannot submit required reporting on their behalf.
Here’s how Medical Mutual will collect this information:
Medical Mutual Small Groups (1-50)
Outreach Method: U.S. Mail
- Small Groups will be asked to provide 2023 plan year information.

- Note: Groups should submit 2024 plan year information following the directions in their 2024 renewal packet.
Medical Mutual Midsize and Large Groups (51+)
Outreach Method: Email
- Depending on when their 2024 renewal was/will be released, Midsize and Large Groups will be asked to either provide 2023 plan year information only OR both 2023 and 2024 plan information. For example: Large Groups in a multi-year renewal will be asked for 2023 and 2024 information.

- Note: If a group’s 2024 renewal packet includes the CAA form, they should complete that form
to provide 2024 plan year information.
Mutual Health Services (MHS) Groups
Outreach Method: Email
- MHS groups will be asked to provide 2023 plan year information.

- Note: Groups should submit 2024 plan year information using the MHS CAA form that is included
in all MHS 2024 group renewal packets.
