Dental, Vision and Life Insurance Options Are Available Outside of MEWA Plan Renewals
SummaCare is a one-stop shop for your MEWA clients. Add dental, vision and life options — even outside of renewals.
Dental: SummaCare offers four dental plans — two with orthodontic options and one with orthodontia built into the rates.
Vision: SummaCare offers two plans with two network options with contributory and voluntary rates.
Life: SummaCare offers four life and AD&D benefit options for employee coverage.
Tax Forms for MEWA Groups
Employers enrolled in the Chamber Health Benefits Plan (MEWA) are required to file form 1094-B with the Internal Revenue Service (IRS). Employer groups are also required to provide the 1095-B form for all employees covered anytime in 2023. For the 2023 tax year, SummaCare is preparing 1094-B (employer) and 1095-B (employees) forms for groups covered during the 2023 calendar year. The tax form PDF packet is now available on the Plan Central Employers Tab. Look for the reporting tab to view the document.
If your client does not have an active Plan Central account, employers can sign up via the Plan Central tab on summacare.com and completing Employer Registration or by contacting ExpressLink.
